How apt that after a discussion about stunt casting, comes a stunt title. Oh Moff, you publicity hungry executive producer you.

If the above is correct, about the regenerations, then it makes sense to automatically think of River Song (wife or not).
Remember, Moff loves his Time stuff and so far River's stories have been happening in reverse order. So, the character in Gaiman's episode that "may well turn out to be a familiar character (Suranne Jones), with a new face" will actually turn out to be River Song and regenerate into Kingston.

Whether we or the Doctor know until the mid season cliff hanger... well we shall see.

The Daily Mail won't be happy that they've spent all that money shooting fake episodes beyond the first two with them all in it. (Hell, all four of them were shooting together ths weekend.)

Given how predictable Moffat is I'm guessing that it's the Doctor with the beard from the Trailers, who's not the "present" version but from the "future". So more Timey-Wimey nonsense.

And Amy's "big secret" is that she (accidently) kills him with the gun we've seen her waving around.

River Song - already killed on screen once - so killing her again would pointless to the existing narrative...

Rory - already killed on screen once - can he be killed twice, and if so why...

The Doctor - major bit of recasting would need to be done on the quiet, would mean the fourth Doctor in just six years

I'm sure Karen is leaving this year, since she has two films lined up to be shot, but she doesn't go in the first two episodes, so cop-out ahoy.

You know if your heart stops, technically you are dead.

Start that heart up again within 3 or 4 minutes and you should suffer no brain damage.

Its very easy for someone to die in the first episode and still be in all the others.
